Form W-9 - Request for Taxpayer Identification Number and Certification is required to be completed by all non-payroll workers.
Required Information
The form requires the worker's information - name and address, and taxpayer ID number - either a Social Security number or Employer ID Number.
Failing to provide the information requested, or submitting false information, can mean that the worker's contracting employer may be required to withhold taxes (at 28%, currently) from all payments.
