Form 8109 - Tax Deposit Coupon

Definition:

Form 8109-B - Federal Tax Deposit Coupon is used to track deposits for payroll taxes, corporate income taxes, and other tax deposits.

The 8109 coupon requires the filer to:

  • Include the tax year ending month
  • The amount of the deposit
  • Identifiers of the business, including the Employer ID Number
  • Check the appropriate box for the type of tax deposit being made
  • And indicate what tax period (quarter) the deposit is being made for.

Tax deposits may be made on this form for:

  • Form 941 - Employer's Quarterly Federal Tax Return
  • Form 940 - Employer's Annual Federal Unemployment (FUTA) Tax Return
  • Form 944 - Employer's Annual federal Tax Return
  • Form 1120 U.S. Corporation Income Tax Return
